*Annual plan fee is nonrefundable. Payment for treatment is due at the time services are rendered unless other arrangements have been made. Plans are non-transferable. Plan cannot be used with any other insurance or discounts. Discounts are applicable to treatment at LS Smiles and cannot be applied to treatment by a specialist. LS Smiles plan is a discount plan not a dental insurance policy. Orthodontics, sleep apnea, cosmetic veneers, and full mouth reconstruction are excluded from this program.
